List of Requirements

  • 1. SEC / DTI Registration Certificate
  • 2. Articles of Incorporation (For Corporation) GIS (For SEC Registered only)
  • 3. Bureau of Domestic Trade Certificate (If applicable)
  • 4. PCAB License Certificate (For Contractors) N.A.
  • 5. Owner’s / Treasurer’s Affidavit (at least 60% Filipino Ownership)
  • 6. Company Profile and Organizational Set Up
  • 7. Latest Business / Mayor’s Permit
  • 8. Pictures of Principals and Alternates
  • 9. Sponsoring Member: _______________________________
