- SEC/ DTI Registration Certificate Articles of Incorporation (For Corporations)
- Bureau of Domestic Trade Certificate (If Applicable)
- PCAB License Certificate (For Contractors)
- Owner’s/ Treasurer’s Affidavit (at least 60% of Filipino Ownership)
- Latest Business/ Mayor’s Permit
- Pictures of Principals and Alternates
